PRIVACY OF PERSONAL HEALTH INFORMATION
De Novo is committed to protecting its clients' privacy. We have information practices in place which comply with applicable laws to protect the privacy of our clients' personal health information. This notice describes the purposes for which we collect, use and disclose your personal health information.
De Novo collects, uses and discloses personal health information to provide inpatient/outpatient treatment services regarding alcohol and drug dependency to its clients. We also collect, use, and disclose personal health information for purposes related to the provision of inpatient/outpatient treatment services. Such related purposes include: Admitting and assessing clients, developing treatment plans, after-care plans and counseling strategies; providing medical care; assisting clients to apply for benefits; referring clients to agencies and services; complying with legal obligations; and seeking treatment related assistance from external professionals.
In addition to the purposes described above, De Novo uses and discloses your personal health information as authorized by law. The law authorizes us to use your personal health information for purposes which include: for planning, delivering, evaluating or monitoring our programs and services, for detecting, monitoring or prevention of fraud, for risk management and for educating employees to provide health care.
Unless you tell us not to, upon request, De Novo may disclose general information about you such as your location in the facility and your health condition. If you are unable to consent personally, we may also disclose your personal health information for the purpose of contacting a relative, friend or substitute decision maker.
Unless you tell us not to, if you give us information about your religious or other organizational affiliation, we may give your name and location in the facility to a representative of the religious or other organization.
De Novo does not use its clients personal health information for research purposes or for fundraising or marketing purposes.
